New patient forms will be emailed to you and must be completed and delivered BEFORE your first mental healthcare appointment! Failure to complete forms may result in your appointment being canceled or rescheduled. There are no refunds on payments for appointments canceled or rescheduled due to lack of completed paperwork.
Please begin with filling out the registration forms that were sent to you via email. Our staff will instruct you on when to proceed to the next step.
At times our staff may direct you to update your information. An email will be sent to you for completion if needed.
If you need a release of information or any other forms please make your request through the patient portal.
