Fee & Insurance Information

All of our appointments are VIRTUAL and completed in real-time video.

We are able to accept most Medical Mutual, Blue Cross Blue Shield and Cigna/Evernorth plans.

We are unable to accept Medicaid or Medicare plans at this time. We also offer self-pay options.

Self Pay Rates

New Patient Visit (up to 60 min) $260

Follow up Med Check (under 10 min) $50

Follow Up Visit (up to 20 min) $105

Follow Up Visit (21 to 30 min) $125

Follow Up Visit (31 to 40 min) $145

Follow Up Visit (41 to 50 min) $165

Follow Up Visit (51 to 60 min) $185

Anything above 60 min add $20 for each 10 min increment

No Show $105


Specialty Service Appointment Fees May Vary.  

Cancellation Policy:

For new patient appointments, we will require a credit card on file. You may cancel up to 48 hours in advance of your appointment and your credit card will not be charged for any no-show fees. If you cancel your appointment with less than 48 hours' notice or do not show up, you will be charged the full amount of the appointment.


Patient Behavior Policies: 

HHH does not tolerate disrespectful behavior towards its staff and providers. All patients are expected to conduct themselves in an appropriate manner through all forms of communication (email, phone, video, or in person). Failure to comply with this policy will result in immediate termination from the practice.
